Note: There are significant errors recorded in the Ohio death certificate for John Henry Moomau. It lists his father as Levi Moomau (Henry) and his birth date as 6 Feb 1863 (1857). It also lists his birth location as Illinois(Ohio) which is also wrong. The only thing that is correct is his death date. The informant was "Mrs. George Moomau" who I presume was the wife (Ida) of John's son, George Albertis Moomau.
