Who was born the 12 day of Dec A.D. 1842 in (blank). County of Belmont. State of Ohio and first entered the service the 9 day of November, 1861 going into Camp Buckingham near Mansfield, O. as a private in Compy H, Capt Brown 65 Regt Ohio Vol. Inf. Was detailed as assistant hospital cook in which capacity he served most of the time of his enlistment.
He was mustered out of the service the 27 day of Nov., 1862 by reason of surgions certificate of disability at camp near Nashville, Tenn.
He names as some of his intimate associates, Comrades Geo. Brestter; Chief Hosp. Cook - Pope; hospital nurse, Dr. Kyle of Xenia, O. and Theodore Ward a cousin.
